Daycare receipt for child care payments (free PDF)
Parents are often asked for child care receipts — by an employer’s dependent care account, by a tax preparer or simply for their own records. Fill in the provider, the parent, the child, the dates of care and the amount, then download a PDF with a signature line for the provider and the parent.

Use it for: child care you have actually been paid for — a home daycare, a nanny, a babysitter or a day camp — by the day, the week or the month.
Not for: an official tax form or year-end statement from a daycare center, care that was not provided, or an amount higher than what was really paid. Whether a plan or a tax office accepts a receipt is up to them, not to the receipt.
What to write on a daycare receipt
Whoever looks at a child care receipt later — the parent, a dependent care plan, a tax preparer — looks for the same details. The form above asks for:
- the provider’s name — the daycare, the nanny or the babysitter — and address;
- the name of the parent or guardian who paid;
- the child’s name in the description, with every child if one payment covers siblings;
- the dates of care: a week, a month or specific days, and the rate if it helps (“4 weeks at $275”);
- the amount and how it was paid: check, cash, transfer or Zelle / Venmo / Cash App;
- the date the money was received;
- a signature line for the provider, and one for the parent.
Dependent care plans and tax forms often also ask for the provider’s taxpayer identification number. If the provider agrees to share it, it can go under More options → Notes; many providers prefer to give it to the parent separately. Registration fees, late pick-up fees or meals belong in the description, so the total matches what was paid.

Daycare receipt template for home daycares
Home daycares and small centers usually give a receipt with every payment, or one at the end of each month that covers all the weeks. Use the same layout every time: same provider name, same wording for the child and the period, so a parent can line the receipts up later without guessing.
Numbering the receipts helps too. Put your own number under More options — for example the year, the month and the family (2026-10-07) — so you can find the matching copy quickly when a parent calls.
Babysitting receipt for sitters and nannies
Babysitters and nannies are often paid in cash or through Venmo or Zelle, and many never write a receipt until a parent asks for one. A babysitting receipt takes a minute: the sitter’s name, the parent, the child, the dates and hours, the rate and the total.
If you sit for the same family every week, write one receipt per payment and keep a copy. Parents who need proof for a dependent care account usually submit the receipts as they go, so a receipt at each payment is easier for them than one at the end of the year.
Receipts for a dependent care account or tax time
In the United States, parents who use a dependent care flexible spending account or claim child care costs on their tax return are often asked for receipts showing who provided the care, when, and how much was paid. What a plan or the IRS accepts is up to them; a receipt shows what was paid, nothing more.
In Canada, parents are often asked for child care receipts at tax time, and when the caregiver is an individual rather than a center, those receipts commonly show the caregiver’s Social Insurance Number. Choose CAD as the currency. Whether an expense can be claimed is decided by the tax authority, not by the receipt.
One receipt per payment, or one for the year?
Both are common. Many providers give a receipt with each payment and a summary of the year in January; some centers have stopped sending year-end summaries, so parents keep the individual receipts instead. This website makes one receipt at a time — if you need a receipt for a whole year, write the months and amounts in the description and the total as the amount.
Receipt for child care services paid in cash
Cash leaves no record, so write the receipt when the money is handed over, at drop-off or pick-up. Print two copies or send the PDF by text or email the same day, and keep your own copy with the date the money was actually received.

Questions people ask
My child care provider won’t give receipts. What can I do?
Ask again in writing and say which weeks or months you need. If the provider agrees but doesn’t get round to it, prepare the receipts yourself with the real dates and amounts and ask the provider to check and sign them. Pay by check or transfer where you can: one Canadian parent reported that bank transfer records helped when the tax office asked questions.
Do I need a receipt for babysitting services?
Not for every casual evening. But if you pay through a dependent care account or plan to claim child care costs, those often ask for proof of each payment, so it is easier to get a receipt as you go than to collect them at the end of the year.
Can the daycare give one receipt for the whole year?
Many do, usually in January. Some centers have stopped issuing year-end summaries, so ask early, or keep the receipt from each payment. A yearly receipt should list the child, the period, the total paid and the provider’s details.
Is a daycare receipt the same as an invoice?
No. An invoice asks the parent to pay; a receipt confirms that payment was received. Centers that bill families with invoices often send a receipt or a statement once the invoice is paid. This website only makes receipts.
